Sunscreen - Choose It Carefully

The rise of melanoma rates in New Zealand over the years may be related to sunscreen use and vitamin D deficiency. You do need to avoid excessive sun exposure to avoid skin cancers, but it's not as simple as that. UV radiation likely causes over 90 per cent of all skin cancers and also increases the likelihood of cataracts and premature skin ageing and causes immune suppression.

Both UVA and UVB radiation cause genetic damage and promote skin cancers. But because most sunscreens do not block out UVA radiation, skin damage can occur even when sunscreen is applied. In addition to this, high SPF sunscreens can completely block vitamin D production by blocking UVB. Because of all of this, sunscreens may falsely give a sense of safety in the sun.

Australia and NZ have the highest melanoma rates in the world, but also the highest sunscreen usage. The negative publicity over excessive sun exposure over the past few years has resulted in such an efficient block out of the sun that we now have a vitamin D deficiency pandemic. Worldwide, it is estimated to be around 50 per cent of the population. Many melanomata occur on non sun-exposed areas - this could be related to a lack of vitamin D. We know vitamin D helps prevent cancer so research is currently looking at the association between vitamin D deficiency and melanoma.

So, why do we need vitamin D and how does it prevent cancer?

Sunlight is the main source of vitamin D in New Zealand. Many New Zealanders have insufficient vitamin D. Vitamin D controls over 200 genes in all tissues of the body. Its benefits are extensive and include prevention of heart disease, autoimmune disorders, SAD, depression and sleep disorders. Vitamin D also helps melatonin production - this is an antioxidant.

Research has shown that vitamin D is a potent cell growth regulator and reduces metastases (the spread of cancer). Vitamin D has been shown to suppress the growth of melanoma cells in test tubes. In one study, patients with the highest vitamin D levels had a 40 per cent reduction in skin cancer.

Sun exposure reduces the risk of certain other cancers, including non hodgkins lymphoma, breast cancer (by 50 per cent), prostate cancer and colon cancer. One researcher reports that a lifetime's sun exposure is associated with a lower risk of melanoma. And that sun exposure is associated with increased melanoma survival. This is also evidenced by the finding that malignant melanoma patients have been found to have lower vitamin D levels. Places with less sunlight in the world have been found to have increased cancers of breast, colon, and prostate cancers.

At last, everyone has come to their senses and have suggested sensible sun exposure in New Zealand that enables vitamin D production. The major groups in New Zealand include: The Cancer Society, The Health Sponsorship Council, The Ministry of Health, the NZ Dermatological Society, Osteoporosis NZ and NIWA. Collectively, they have published a "Position Statement about the risks and benefits of sun exposure in NZ".

In summary:

Sensible sun protection behaviour should not put people at risk of vitamin D deficiency.

The position statement's recommendations summarised:

  • Avoid sunburn - the worst risk for skin cancer is when you are sunburnt as a child then get low vitamin D as an adult.
  • Some sun exposure is recommended for vitamin D production - it is best to expose larger areas briefly than smaller areas over longer periods. Darker skins need more exposure though it's unsure how long.
  • Extra measures to get adequate vitamin D levels are needed in those at increased risk of vitamin D deficiency. Eg. the elderly, the obese, and those suffering from certain illnesses.
  • Those at high risk of cancer should discuss their vitamin D needs with doctor. They need to avoid sun exposure.
  • More research is needed for definite advice about times in the sun.
  • Solariums are dangerous and may emit much higher concentrations of UV radiation than the sun.

Vampire Facelift® Procedure

This procedure addresses the 3 signs of the ageing face:

There are 3 specific steps in this procedure:-

  1. Hyaluronic acid dermal filler is injected in a specific way to sculpt a more youthful but natural appearing face. To be able to do this, Dr Pitsilis takes into account the specific mathematics of beauty.
  2. Two teaspoons of your blood is drawn and specifically prepared to extract and activate the platelets which extrude multiple growth factors when injected into your tissues. This stimulates increased collagen, fatty tissues and blood flow to create younger appearing skin.
  3. Using numbing cream and a very small needle and a specific microneedling technique, your own growth factors are injected back into your face in a particular way. These growth factors then activate your multipotent stem cells within your skin to stimulate younger looking tissues.

Priapus Shot® (P-Shot®) Procedure

This procedure addresses the problems of :-

The common problem of Erectile Dysfunction increases with age. At age 45 years, 50 % of men suffer from erectile dysfunction and it worsens from there, worsening with added disease and drugs.

This is a serious problem that inevitably affects the confidence and self esteem of men.

The P-Shot® procedure involves:-

  1. Numbing cream is applied to the penis by patient
  2. Two teaspoons of blood is drawn
  3. Activated Platelet Rich Plasma is produced, full of growth factors.
  4. Your PRP is injected into the penis using a tiny needle into the numbed penis in a way that distributes the growth factors in a proper way. (five tiny injections)

Questions and Answers about the P-Shot®

Q. How does the procedure work?

A. The Platelet Rich Plasma contains growth factors that stimulate tissue healing and growth. PRP stimulates improvement of the penile blood vessels and other tissues in the penis. PRP also stimulates the local tissue stem cells which are activated to rejuvenate and restore the tissues as well. The overall result are penile tissues restored to a more youthful state.

Q. What is the usual response to the P-Shot® injection?

A. The initial increase in volume often observed is due to the actual volume of PRP and any volume of growth factors and any water associated with them.

Over the next few days some of the water is absorbed by the body. After that, there is growth of new penile tissue over the next few weeks that increases the volume to a level close to what it was with the initial injection. Some men have some, but less growth. Results are variable.

Smokers, those with chronic illness and older men have the least growth and the least response.

Q. How many treatments do I need?

A. There is one treatment. It may be repeated within a few months for a boost if desired. A second treatment should be planned for.

Q. How effective is it?

A. Over 80% of healthy men have an immediate and then a long lasting result. Around 20% notice and increase in girth and length.

Q. How much growth can I expect?

A. In some men, there is to 20% increase in length and girth. Girth seems to occur first, then length with continued growth. It is not yet known if a second injection at 4-8 weeks will cause another growth spurt.

Q. Is there a risk of scar tissue forming?

A. No. This is more likely with the multiple injections of drugs used regularly for erectile dysfunction. In this case, the injections are very infrequent and the risk is virtually non existent. There has been no scar tissue seen with injections of PRP to the face.

Q. Can I react to this injection?

A. No. PRP has been administered to patients for decades within many medical disciplines from dentistry to orthopaedic, to musculoskeletal treatments and it has never been found that a person has reacted to their own cells within the PRP.

Q. Does it hurt?

A. The procedure is quite brief, and the penis is numbed with cream. It's a very small needle. The pain is very minimal.

Results associated with P-Shot® procedure may include a general rejuvenation of the penis. It improves the blood vessels (microcirculation) and tissues of the penis.

The penis may, in some, grow in girth and length (erect and non- erect) with time, as well as become firmer during erections.

There may be improvement in Penile Sensation and Sensitivity, Erectile Firmness, Quality and Stamina and Orgasmic Quality.

There will also be straightening of a bent penis.

Results may vary.

This treatment can also be seen as a maintenance treatment for healthy erectile function so that with ageing, the erectile tissues and blood vessels can be maintained rather than diminished.
